What is H2H?

Hospital to Home (H2H) is a client orientated program designed to provide support for mental health consumers during hospital admission and the 6-8 week period after discharge. This is a time when many consumers are vulnerable and require additional support in order to return to living fulfilling and autonomous lives. H2H focuses on helping consumers to self-manage their recovery, connect with their social networks and minimise feelings of isolation.

  • Reduced return to hospital rates
  • Reduced rates of attempted or completed suicide

How is H2H funded?

The H2H program is supported by the Australian Government through Partners in Recovery (PIR) by different lead agencies. In Western Sydney the program is funded by WentWest Limited, the Inner Western program is funded by New Horizons, and the North Coast program is funded by Mission Australia.

Who are the Support Workers?

Support Workers have lived experience with mental illness and also have relevant qualifications within the mental health sector. This combination equips our Support Workers with the ability to both empathise with individuals experiencing mental health challenges whilst providing informed support and interventions.

What will Support Workers Do?

  • Will establish initial contact with the person while in hospital
  • Will maintain regular scheduled visits as agreed
  • Will participate in the discharge planning process
  • With consent from the consumer, engage with carers, families and others in the consumer support plan
  • Will provide practical support such as initiate referrals, source information, assist with paperwork and assist to ready the home environment such as having utilities and food available
  • Will arrange transport to reconnect with community and maintain follow up appointments
  • Will help the consumer to establish and achieve goals
  • Will be there just for a chat!
